It's easy... Here is the way our 'Share Your Smile'  Referral Rewards Program works:  

If you are a current patient of Valley Dental Center and would like to recommend us, just have your referral fill-out the referral certificate and call 309-799-3000 to make an appointment. At the conclusion of the appointment, they will receive a $20 Gift Card*.

And, as a thank you for your referral, we'll also present you with a $20 Gift Card when you arrive for your next appointment. For each new adult patient you refer to us, you and the new referral will each receive a $20 Gift Card.  

To get started, just click on the above Referral Rewards Certificate, print out the number of certificates needed, and give these to your family and friends.

The new referral must present the completed 'Share Your Smile' Referral Rewards Certificate to us at the initial appointment to receive their reward and so that you are appropriately rewarded for the referral.

There is no limit to the number of $20 Gift Cards you can receive. So, 'Share Your Smile' and reap your referral rewards!
